I have heard of fluoride treatment at the dentist’s office to help prevent cavities. What is fluoride varnish?

Fluoride varnish is a type of topical fluoride that is "painted" on the teeth and dries quickly. The 2.36% strength varnish has been shown to be very effective in preventing cavities. Cavity varnish is rapidly replacing the 1.23% fluoride gel as the procedure of choice for topical fluoride cavity protection. Almost all patients like the varnish much better than the gel for two main reasons. First, the varnish is quickly painted on teeth with a small brush, while the gel is delivered in a big foam tray that has to be in a patient's mouth for 4 minutes. Additionally, patients can eat 15 minutes after the varnish is placed, but with the gel, it is best to wait several hours before eating to maximize its preventive benefits. My hygienist routinely recommends varnish during orthodontic treatment to prevent cavities.

My 8 year old son grinds his teeth at night. Should I be concerned?

A recent study in The Journal of Sleep Medicine answered this question. Almost half of all first graders in the study had parents reporting that their child grinded his or her teeth at least one time per week. This study found that grinding (bruxing) was not associated with breathing disorders during sleep or snoring. The study did find that there was an association with increased internalizing behaviors such as being depressed, being withdrawn or having anxiety. Chronic, frequent bruxism was also associated with increased health problems and decreased neurocognitive performance (learning). Grinding once a week is probably not a reason for concern. Grinding a few times a week should be checked out.

My teeth are sensitive to hot and cold sensations. What causes this problem?

Tooth sensitivity can be caused by a number of reasons. Some of the more common culprits are cavities, cracked teeth, broken fillings, grinding your teeth, and receding gums. Everyone has microscopic tubules in their enamel that connect with the nerve of a tooth. If these tubules are exposed, temperature changes from eating hot or cold foods are transmitted more easily too the nerve. If your sensitivity is from a cracked tooth or grinding, then the changes in pressure on the tooth are causing your sensitivity. My dentist told me that one of my child's baby teeth is stuck to the bone and is delaying eruption of the permanent tooth. Is this a difficult problem to correct?

A baby tooth stuck to the bone is called being "ankylosed", and it can complicate the eruption of the permanent tooth. This baby tooth should be closely monitored. An ankylosed baby tooth usually does not fall out on its own. It usually needs to be extracted. The baby tooth should be removed if the eruption of the permanent tooth is delayed by six months or more. The baby tooth should be extracted as soon as possible if the permanent tooth underneath is ready to come in and if there are developing bone defects or significant changes in the bite. I have braces. Is it better to use a manual tooth brush or an electric toothbrush?

A recent study concluded that an electric toothbrush was "superior" to a manual toothbrush for plaque removal. The study found that the electric toothbrush motion more effectively removed plaque than the manual scrubbing action. Plaque is notorious for developing around braces, so in addition to recommending electric toothbrushes, I use the Damon System of Braces to help make cleaning easier. Damon braces are smaller than traditional braces and don't have wire ties or elastic rings that collect plaque. I recently had four teeth removed to fix my crowded smile. I trusted the both the person that recommended me and the person that treated me. I found out today that I could have been treated without having teeth removed. Why wasn’t I offered this option at the beginning?

There are often different ways to correct crowded teeth. Taking out teeth or not taking out teeth is a big decision that needs to be resolved before starting treatment. If keeping all of your teeth is important to you, don’t be afraid to express that to your healthcare provider. If you bring up your concerns, then your provider should discuss all of the options available. Are all braces and wires used to straighten teeth the same?

No, just like many of the products you buy, there is a wide difference in the equipment and technology used by orthodontists. Traditional braces use a metal wire tied together with smaller wires or elastic bands. The newer self-ligating bracket technology uses “doors” on brackets to hold the wire in place. This wire is secured in a loose and comfortable manner know as passive ligation. My wife says that I seem to have trouble breathing when I sleep. Can this cause me long-term harm?

A recent study found that Obstructive Sleep Apnea (OSA) can take off years from a person's life expectancy. OSA is the cessation of breathing while sleeping because the path of airflow is blocked during respiration. It is estimated that more than 18 million Americans have OSA. Some of the more common signs or contributing factors are: snoring, being overweight, hypertension, diabetes, age, increased neck size (size 17 males and 16 females), morning headaches that soon disappear after waking up and constantly being sleepy during the day. I want to get my teeth straightened and I want to bleach them to make them whiter. Should I do it before or after treatment? How does bleaching work?

If you are using Invisalign aligners to straighten your teeth then it can be done before, after or even during treatment. If you are getting braces, after is best. The active ingredient in bleaching gel is hydrogen peroxide. Hydrogen peroxide is applied to your teeth using a clear plastic tray. As the hydrogen peroxide degrades it produces free radicals that diffuse rapidly through enamel and dentin. These free radicals are strong oxidizers that split up pigmenting compounds that diffuse out of your teeth, causing the tooth to absorb less light. This process causes teeth to look lighter. Q: My dentist wants me to get a second opinion from you because the first orthodontist I went to wants to pull some of my teeth. Why do I need a second opinion? Aren't all orthodontists pretty much the same?

Dr. Herman's Answer: Not all orthodontists are the same. We differ based on where and when we trained, our years of experience and the number of patients we have treated. Q: Can I chew gum if I get braces?

Dr. Herman's Answer: Yes! I encourage my patients to chew gum, but it must be sugar-free gum. Several studies have shown that chewing gum after orthodontic appointments decreases initial discomfort. Sugarless gum does not increase the risk of damage to braces. Also with the Damon System of Braces, eating popcorn is permitted. The smaller brackets and the elimination of the tie wires prevent popcorn from getting stuck. You still cannot eat popcorn if you have the larger traditional braces. Q: I see many of your articles about the Damon System of Braces but none on Invisalign. Why?

Dr. Herman's Answer: I like Invisalign and I use Invisalign when my patients request it. The advantages of Invisalign are that it’s clear, removable and patients do not have any food restrictions. The disadvantage is that Invisalign cannot correct severe orthodontic problems, so not everyone is a candidate for Invisalign. Also, it’s the responsibility of the patient to wear the aligners as directed and not misplace the aligners. Many patients, especially teens, have trouble maintaining the discipline needed to have a successful treatment. Q: I am 43 years old and I don't like my smile. I want to get braces to fix it. Are there many adults like me wanting braces?

Dr. Herman's Answer: The American Association of Orthodontists recently surveyed 1,000 adults ages 18 and older. This survey found that 1/3 of the adult respondents were unhappy with the way their smiles looked. In the sample, 36% of respondents felt that a better looking smile would result in a better social life. 76% of the respondents said that people with crooked teeth were perceived to be less successful than people with nice looking smiles. Lastly, 14% of those people that did not like the way their smiles looked thought that it caused them to miss out on getting a better job. Q: I read an article that stated the amount of periodontal disease among adults over age 30 was worse than anyone thought. I also heard more adults are getting braces. Are these two facts related?

Dr Herman's Answer: The study found that 47% of adults over age 30 have some form of periodontal disease. Mild, moderate and severe rates were 8.7%, 30.0% and 8.5% respectively. The AAO and AAP put out a joint recommendation that undiagnosed periodontitis can worsen with orthodontic treatment, and orthodontists should be prepared to deal with this complication. For this reason, I have a periodontal health department within my orthodontic practice. I am prepared to diagnose and treat most periodontal conditions before or during orthodontic treatment. Q: I had four teeth taken out as a teen when I had braces. I am now 40 years old and I snore. My dentist says that the jaw holding my teeth is too narrow and needs to be widened in order to reduce my snoring. Are there any other side effects to having teeth removed?
