I visited another orthodontist who wanted to put screws into my bone to fix my bite and said that a narrow smile was more stable than a wide smile. I like the look of a wide smile. He also told me I would need to wear retainers after braces to keep my teeth straight. If a narrow smile is more stable, then why do I need retainers?
There are no studies that show narrow smiles are more stable than wide smiles. Most patients prefer a wide smile because of the positive impact it can have on their overall appearance. I just got a mail advertisement from an orthodontist. It features a patient who says she got braces at age 7 to correct her cross bite. Is age seven too early?
It is best to correct a cross bite early. There are two types of cross bites: anterior cross bite, better known as an under bite, and a posterior cross bite. A posterior cross bite is when the upper back teeth bite inside of the lower back teeth. It can be on just one side or on both sides. The goal is to correct the cross bite in a year or less through braces, take off the braces, and let the child resume growth for another year or two. I find that most cross bites can be corrected with upper braces only. I correct almost all cross bites without using invasive palatal expanders. I took my child to an orthodontist for a consultation. Before my son saw the doctor he had two x-rays taken, a panoramic x-ray and a cone beam x-ray. I didn’t think this was necessary. Is that typical?
As a parent, I would also express concern with two x-rays during an initial consultation. I understand the need for a recent panoramic x-ray, as that is typically needed for exams. My concern is unnecessary exposure to radiation. I believe a cone beam x-ray or a cephalometric x-ray should only be taken after the initial exam if the doctor feels it is necessary. There should be a difference between protocol for an initial exam and protocol for final diagnosis records. The latter is done after the doctor determines there is a need for additional records and has discussed it with the parent.

I’ve heard that you very rarely remove teeth to correct crowding. Why?
Many orthodontists remove teeth to make room for crowding because it is a relatively easy way to make space. After years of observing this practice, I agree with many other orthodontists that it negatively impacts the profile of the face. Their faces can seem sunken in, their noses can seem longer, their lips can look thinner, and the chin sticks out more. I know that you offer high-tech braces. Why do some offices offer both traditional braces and high-tech braces?

I hear great things about you, your staff and your results. I want to be clear; I do not want my daughter to get extractions, screws in her bone or an expander that rips her bone apart. I have already gone for a consult elsewhere and do not agree with any of these methods. Can you accomplish treatment without using these devices or removing teeth?

I recently had four teeth removed to fix my crowded smile. I trusted the both the person that recommended me and the person that treated me. I found out today that I could have been treated without having teeth removed. Why wasn’t I offered this option at the beginning?
There are often different ways to correct crowded teeth. Taking out teeth or not taking out teeth is a big decision that needs to be resolved before starting treatment. If keeping all of your teeth is important to you, don’t be afraid to express that to your healthcare provider. If you bring up your concerns, then your provider should discuss all of the options available. Are all braces and wires used to straighten teeth the same?
No, just like many of the products you buy, there is a wide difference in the equipment and technology used by orthodontists. Traditional braces use a metal wire tied together with smaller wires or elastic bands. The newer self-ligating bracket technology uses “doors” on brackets to hold the wire in place. This wire is secured in a loose and comfortable manner know as passive ligation. Q: My dentist wants me to get a second opinion from you because the first orthodontist I went to wants to pull some of my teeth. Why do I need a second opinion? Aren't all orthodontists pretty much the same?
Dr. Herman's Answer: Not all orthodontists are the same. We differ based on where and when we trained, our years of experience and the number of patients we have treated. Q: Can I chew gum if I get braces?
Dr. Herman's Answer: Yes! I encourage my patients to chew gum, but it must be sugar-free gum. Several studies have shown that chewing gum after orthodontic appointments decreases initial discomfort. Sugarless gum does
not increase the risk of damage to braces. Also with the Damon System of Braces, eating popcorn is permitted. The smaller brackets and the elimination of the tie wires prevent popcorn from getting stuck. You still cannot eat popcorn if you have the larger traditional braces. Q: I see many of your articles about the Damon System of Braces but none on Invisalign. Why?
Dr. Herman's Answer: I like Invisalign and I use Invisalign when my patients request it. The advantages of Invisalign are that it’s clear, removable and patients do not have any food restrictions. The disadvantage is that Invisalign cannot correct severe orthodontic problems, so not everyone is a candidate for Invisalign. Also, it’s the responsibility of the patient to wear the aligners as directed and not misplace the aligners. Many patients, especially teens, have trouble maintaining the discipline needed to have a successful treatment. Q: Is there any research supporting the Damon System of Braces claim to have less discomfort (pain) than other types of braces?
Dr. Herman's Answer: A recent study gave some more insight to this question. The study was on friction within several self ligating types of braces. These types of braces don’t use wires around the brace to tighten. The study measured “kinetic friction” or friction between the braces (bracket) and the wire. The study found that the passive self ligation system by Damon had, by a large amount, the lowest amount of friction. This is important because friction can make the force used by an orthodontist to move teeth to be lost in overcoming friction rather than moving teeth. So an orthodontist increases force in high friction systems. To the patient more force usually means increased soreness (pain).
